About Diana
Diana Falise is a certified family nurse practitioner with a strong foundation in primary care and a passion for patient education. Her career has included work in rural and underserved communities—experiences that have shaped her deep commitment to improving access to high-quality care for all individuals, regardless of location.
With dual undergraduate degrees in nursing and child and family development, Diana brings a unique, whole-family perspective to her practice. She specializes in primary care across the lifespan and emphasizes patient-centered education as a tool for empowering individuals, families, and communities to take charge of their health and wellness.
Diana believes that strong provider-patient relationships are the cornerstone of effective care. Her approach is warm, collaborative, and rooted in listening—she works with patients to create personalized care plans that meet their goals and support long-term wellbeing.
Diana recently relocated to Oregon and is excited to explore the Pacific Northwest alongside her family. Outside of work, she enjoys gardening, discovering new foods, attending live music, the arts, and getting to know her new community
